Natasha Adams, a mother of three, was found dead in her home hours after a mental illness crisis team reportedly failed to show up for an emergency appointment.
Family members said community mental health workers will be visiting their home on 12th August, BirminghamLive reports.
However, NHS staff did not show up and did not arrive until hours after his body was found by a neighbor the next day, his family claimed.

What Was The Incident?
She was from Solihull and went to the emergency room at Queen Elizabeth Hospital in Birmingham on 11th August for help with her severe anxiety.
She is believed to have seen employees from Solihull and Birmingham Mental Health Hospital Trust based at the site.
Her family claim they were told that psychiatric staff at the hospital made arrangements to see Natasha at her home on Thursday.
But they claim they never showed up, they only arrived a few hours after her body was discovered on Friday.
Heartbroken Marie said ‘Tasha’ was on a family holiday in Scotland with her parents and children when she decided to return to Birmingham.
Late on Wednesday, August 11, he went to QE Hospital for help with severe anxiety.
Marie von Shirley said: “She went to the emergency room. The hospital said it would make sure the home mental health crisis team came to see her the next morning.”
“So Tasha left the hospital around 1:30 am Thursday. I think he was there for about three hours.”
“After Tasha’s death, the crisis team informed me that they wanted to visit her on a Thursday morning. They said,” We’re really sorry, there will be an investigation.
“You sent someone to see Tasha for the first time on Friday 13th August at 11 am when she was already dead.”
Natasha was the eldest daughter of parents Marie and Steve, who have five other children.
Family Statement
Her mother said the former hairdresser suffered from mental health issues since school when she also needed heart surgery.
Marie said: “She was bullied at school, so she left school early.”
“He had heart surgery when he was younger and I think the bullying and heart surgery triggered his mental health issues.”
“He didn’t finish his last two years at school and then went to Solihull College. She learned math, English and eventually went to college, and when pregnant with her oldest son, she graduated as a level 3 hairdresser.”
“She did this for a while and she loved it. All of her friends got free haircuts, hairstyles, and coloring.”
But mental health issues continued into adulthood, and Natasha was finally diagnosed with bipolar disorder in 2020.
Her mother said, “There have been terrible ups and downs, terrible ups and downs. I resumed the conversation with the doctor because he said everyone has their ups and downs.”
“When she was high, they were happy highs. He could go to bed with the minimum, but the school continued to function.”
But he always put a smile on his face when he saw people. She has literally helped so many people with mental health issues.
“There was a young woman that Natasha sat with for 13 hours because this young woman was suicidal. This young woman told us that without Natasha she wouldn’t be here with her children.”
Natasha’s three children, aged one to seven, who were destroyed on the ground, are now in the care of their parents.
